What is CHADIS
It is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office.
How it Works
- After you complete the questionnaire, CHADIS results come directly to your doctor.
- The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.
- We use CHADIS for all of your child's checkups, at all ages, and additionally use it for behavioral and mental health
New Users
REGISTER for CHADIS- Enter our office phone number as the invitation code
- Choose a username - your email address works well
- Record password, you will access CHADIS prior to each well visit
Returning Users
CHADIS will now be completed through your patient portal. Please call the office if you do not see your assigned questionnaires for an upcoming visit.
